CalAIM
California Advancing and Innovating Medi-Cal (CalAIM) is a far-reaching, multiyear plan to transform California’s Medi-Cal program to work more seamlessly with other social services.
Led by California’s Department of Health Care Services (DHCS), the goal of CalAIM is to improve outcomes for the millions of Californians covered by Medi-Cal, especially those with the most complex needs. As a CalAIM program provider, Youth For Change offers case management services to deliver Community Supports and Enhanced Care Management to eligible clients.
Community
Supports (CS)
Community Supports (CS) are services that help improve the overall health and well-being of Medi-Cal members by addressing members' health-related social needs, helping them live healthier lives, and avoiding higher, costlier levels of care. These services are provided through case management which supports clients in finding, securing, and maintaining housing, learning independent living skills, and accessing community resources.
Enhanced Care Management (ECM)
Enhanced Care Management (ECM) is a statewide Medi-Cal benefit available to select members with complex needs. Enrolled members receive comprehensive care management from a single lead care manager who coordinates all their health and health-related care, including physical, mental, and dental care, and social services. Enhanced Care Management makes it easier for members to get the right care at the right time in the right setting, and receive comprehensive care that goes beyond the doctor's office or hospital.
How Does it Work?
Community Supports
Once referred, you will be assigned a case manager who can support you in your search for safe, independent housing. If you are experiencing homelessness or at risk of experiencing homelessness, you will receive help to find, apply for, and secure housing. You are eligible to receive housing security deposits, household necessities, and utilities set-up fees. You will also receive support to maintain safe and stable tenancy once housing is secured, such as coordination with landlords to address issues, assistance with the annual housing recertification process, and linkage to community resources to prevent eviction.
Enhanced Care Management
Enhanced Care Management offers access to a single Lead Care Manager who provides comprehensive care management and coordinates your health-related care and services. They provide connections to the quality care you need, no matter where you seek care—at the doctor, the dentist, with a social worker, or at a community center.
Who is Eligible?
CS Eligibility
CS are available for anyone receiving Medi-Cal from Partnership HealthPlan of California and are experiencing, at risk of, or transitioning from homelessness.
Referrals must also have:
One or more qualifying clinical risk factor
Are prioritized for a permanent supportive housing unit or rental subsidy resource through the local homeless Coordinated Entry System
ECM Eligibility
ECM is available to children, Transitional Age Youth (TAY), and families receiving Medi-Cal from Partnership HealthPlan of California who are:
Experiencing or at risk of homelessness (adult & youth)
Birth Equity: Pregnant and postpartum individuals at risk for adverse perinatal outcomes (adult & youth)
Avoidable hospital or emergency department use (adult & youth)
Serious mental health or substance use disorders (adult & youth)
CCS or CCS Whole Child Model (youth)
Foster Care (youth)
Justice Involved (adult & youth)
If eligible, a client may be enrolled in CS and ECM concurrently.
How Long Does it Last?
Community Supports
Community Supports are intended to provide case management and resources to find you stable and independent housing. The initial referral is valid for six (6) months and extensions are available on a case-by-case basis pending approval from Partnership.
Enhanced Care Management
Enhanced Care Management is an ongoing benefit for those who qualify. The initial and continuing referrals are valid for twelve (12) months.
